Exploration of Inner Body Awareness and Anxiety in Adolescent Autistics: A Thematic Analysis Based on Personal Insights

Individuals with autism often have altered interoception, making it challenging for them to recognise and manage internal bodily sensations, such as those associated with anxiety. This heightened sensitivity to sensory input can exacerbate anxiety, creating a persistent sense of threat.
The study, which involved 13 autistic adolescents aged 14-17 from diverse UK backgrounds, used a reflexive thematic analysis using semi-structured interviews. Three main themes emerged: Interoceptive Awareness, Interoceptive Evaluation, and Situational Interoception.
Interoceptive awareness plays a crucial role in emotional regulation, and the presence of trauma can complicate this process, leading to increased anxiety and other mental health issues. The study found that appropriate awareness allowed participants to regulate their emotions without inducing anxiety. However, in some cases, hyperawareness, overly heightened focus on bodily signals, led to overwhelm and anxiety.
Interventions that enhance interoceptive awareness, such as mindfulness exercises or sensory regulation activities, can help individuals better recognise and manage their emotional states, potentially reducing anxiety. The study suggests that improving interoceptive awareness can be beneficial for managing anxiety in autistic individuals.
The findings also highlight the complexity and variability in interoceptive experiences of autistic adolescents and their relationship to anxiety. Different responses depending on bodily signal type (cardiac, respiratory, gastric) were observed under Situational Interoception. For instance, heightened anxiety and exercise increased awareness, forming bidirectional feedback loops under Arousal. Conversely, hypoawareness, limited ability to detect signals, led to delayed anxiety management, intensifying distress.
The study addresses gaps in existing research regarding interoception and anxiety in autistic populations, particularly adolescents. It suggests that targeted interventions could be developed to manage anxiety through addressing interoceptive awareness and evaluation. Potential ethical implications of using wearable technology for managing anxiety in autistic adolescents with hyper-awareness need careful consideration.
